fetch: folder-per-function for top-level modules + sanitize-response rename
Apply the folder-structure rule to the remaining flat src modules — each gets
its own folder with collocated tests:
check-rate-limit/, decide-action/, injection-analyzer/, intercept-fetch/
(incl. path-extraction test), request-store/, validate-url/,
sanitize-response/ (renamed from response-sanitizer; matches its
sanitizeResponse fn).
Kept flat under src/ per request: config.ts, init.ts, audit-logger.ts, index.ts.
Imports to config/audit-logger stay flat (../config.js); cross-module imports
now resolve through the new folders. References updated across claude/, opencode/,
and the package barrel.
195 fetch tests pass, type-clean.
